Our Community Navigation program is designed to help LGBTQ+ older adults access the services and resources they need with personalized support. Navigators work one-on-one with individuals to guide them through complex systems like healthcare, housing, legal services, and social programs, ensuring they receive the care and assistance that is both affirming and inclusive of their unique identities. - – Transportation assistance – Support in accessing safe, reliable transportation.
- – Referrals to affirming providers – Connections to LGBTQ+-inclusive healthcare and service providers.
- – Long-term care navigation – Assistance in evaluating and securing affirming long-term care options.
- – Housing advocacy & navigation – Helping individuals find and maintain safe, stable housing.
- – Emergency food support – Immediate access to food resources.
- – Medicaid counseling – Expert support in understanding and maximizing Medicare benefits.
- – Utility payment assistance – Help with securing energy assistance programs.
- – Vaccination navigation – Support in finding and accessing critical vaccinations.
