Just as the name suggests, Person-Centered Options Counseling (PCOC) –sometimes called ‘options counseling’–is person-centered—meaning it focuses on you, your needs, your goals, and what matters most in your life. It’s also all about options—you lead the discussion, and after learning what’s important to you, our certified PCOC counselors walk you through the range of supports, resources and services available, providing clear, unbiased information so you can weigh your choices and decide what works best for your life.
PCOC can help with things like:
- – Exploring long-term care options at home or in the community, including in-home help, transportation, and social or wellness programs
- – Understanding next steps after major life changes -such as a hospital stay, new diagnosis, or move – so available care and support options are clear
